Live Well - Graduate Program
Helping the families we serve not only achieve success today but also maintain stability in their housing, financial well-being, and career paths for a brighter tomorrow.
Workforce development. Housing partnerships. Financial capability. Health and wellness.
Homelessness has many causes, and housing stability requires many solutions. Our Live Well - Graduate Program supports families who have successfully graduated from the Shelter Program. We provide continued case management, support groups, counseling, financial accountability, and mentoring for up to 24 months after families leave the program. At FPOCC, we are committed to helping our families move forward on their journey to sustainable independence.
Our goal is not simply to get families into housing—it is to keep them in housing.
The key to achieving this lies in identifying the specific needs of our community and tapping into the diverse resources offered by our volunteer pool and partnerships. The result is families who are no longer at risk of homelessness and are equipped to thrive.

Specifically, we can provide:

-
Ongoing case management offering referrals and resources
-
Help with household items, food & holiday help (when available)
-
Financial assistance (when funding available)
-
Use of Day Center computers and printer by appointment
-
Notary services by appointment
-
Volunteer opportunities
-
Budget assistance and classes
Almost Home
In addition, our "Almost Home" program is designed for guests who have successfully completed the shelter program. This program offers temporary, affordable housing for up to 12 months when available to those who have achieved their case management goals. A limited number of homes are available for families to use as they transition from the shelter to a more permanent future residence. For families unable to utilize these transitional homes, financial support may be available to assist with rental and utility deposits and application fees.
Volunteers also play a vital role in our Live Well - Graduate Program. They are needed to provide meals for support groups, assist with children during meetings, or host special events or outings to celebrate successes. Volunteer speakers specializing in housing, credit counseling, parenting, decorating, and other fun, informative topics are also needed for support group meetings and one-on-one interactions.
